All ears are different. It is impossible to predict the size of an ear canal by the appearance of the outer ear, or by a person’s physical size, age or gender. Two sizes of ER20 Plugs fit almost 100% of ear canals. Recent studies of both fit and seal, conducted at Etymotic Research and an independent lab, showed that the smaller of the two sizes fits the majority of ears and is now designated as standard fit.
ADJUSTING TO ETYMOTIC EARPLUGS
A little time may be required to acclimate to these earplugs. Many musicians report that they hear their own instruments better, as well as the balance with those around them.
Insert the plug by grasping the end cap and inserting the eartip until the largest flange is sealed in the ear canal entrance. To ease insertion, moisten the plug and pull the ear outward and upward. Earplugs must be inserted correctly for optimal effectiveness. To remove, use a slow twisting motion.
WHEN TO REPLACE
ER20 Plugs are reusable, but with repeated use, the eartips may lose elasticity. For best performance and retention, if you use your constantly, replace them every 3-6 months.

I was a diehard etymotics triple flange user for years. Always kept them on my keys and had no issues, the fit & case was perfect. I ended up losing one in May (2025) and replaced with these - really wish I hadn't!
The fit is much worse than the triple. I never feel like I'm getting a good seal and even the better fit of the two sizes pops out of my ears periodically. Plus, the case is now much worse quality than the one I had before. The case my triple flange plugs came with was a black silicone material and I NEVER had issues with open/close or with it being on my keys. The new case's chain has already broken and I nearly lost the ear plugs, and now the plastic snap closure seems to be having issues to. Its very difficult to fully close, and I can't tell if its because the plastic already broke/wore down or bad design.
I'm considering going back to the triple flange, but with the case issues might just go elsewhere.
These attenuators are decent, but they don't seal as well as I had hoped. I don't understand why they don't offer a size Large. I'm not sure, but I feel a touch larger would be a better fit. Or softer silicone. I like these better than the 3-flange models. I am 5'10" with a largish head.
I'm writing this to correct my previous review. The dual flange is more comfortable than the triple flange, but the triple flange gives MUCH better hearing protection and blocks a lot more sound.
These are really comfortable, but they don't have nearly the same noise reduction as the ER20XS. I'm going to keep these and put the three flange ear tips on them for better sound reduction.
Comfortable easy to put in and remove from ears well made and 20 db reduction. Using for concerts etc. would buy sgsin
